You will be given many consent forms to read and sign when your child first comes into my care. These consent forms are:

1. Calpol Consent
2. Emergency Treatment Consent
3. Prescribed medication Consent
4. Outings Consent
5. Photograph Consent
6. Sun Cream Consent

You are not obliged to sign any consent for that you are not happy with however please discuss them with me first.
